Crestwood Office
Our Crestwood location is conveniently located just ½ block East of Cicero Avenue on 138th Street. Our Office Manager is on hand at the hours listed below and can handle most of your questions and concerns regarding our office. For your convenience, below is information regarding our location, office hours, appointment scheduling and insurance networks we participate in.
Appointments
Waiting times are minimal and Dr. Gagnon sees patients by their “appointment time” not by the “time they arrive”. If a patient is early, we try to see them as soon as possible but priority is given to patients that arrive on time. If a patient is unable to keep their appointment, we ask they they call us before 9:00 a.m. on the day of their appointment and they can leave a message on our answering machine.
Insurance
We are an Advocate facility and accept Medicare, PPO and HMO health plans, Workmen’s Comp. and Self-Pay patients. To verify we are in-network with your plan, please call any one of our offices: Crestwood (708-385-4520), Orland Park (708-403-7250) or Summit (773-229-8200).
