We welcome new patients at our practice and at your first visit need to collect important background information such as medical history which can affect the health of your feet. We also give you plenty of time to get to know our doctor and get the care you need.

Our “New Patient Form” is available below if you wish to print and fill it out before arriving to your visit. We ask that you arrive on time for your appointment and bring the following with you:

• Insurance card
• A list of current prescriptions and over-the-counter medications, including dose and frequency
• Information about your medical and surgical history
• Recent test results, x-rays, or relevant records

Insurance and Payment Information

We are an Advocate facility and accept Medicare, PPO and HMO health plans, Workmen’s Comp, and offer Self-Pay Options for those without insurance. To verify we are in-network with your plan, please call any one of our offices: Crestwood (708-385-4520), Orland Park (708-403-7250) or Summit (773-229-8200).

All patient copays are due and payable at each visit.
